Trademark Registration

The following information is needed to prepare a trademark application for filing (you should use the TM symbol before you have obtained a federal registration at the U.S. Patent Office)

INFORMATION FOR FILING A TRADEMARK APPLICATION

1. APPLICANT

-if filing in your name, then your full name and home address;

-if filing in your company name, then your company name, address, and state of incorporation

2. Filing WORD mark or LOGO

-if logo, then a jpg image of the logo;

-do you intend to claim any particular color scheme?

3. FIRST USE

-first use in US Commerce month/date/year (actual sale or offer to sell)

4. GOODS/SERVICES

-provide description of goods and/or services

5. SPECIMEN/PROOF of USE

-if already started using in commerce, then a jpg photo of the trademark on the actual product or packaging or label (for products); or

-a pdf copy of a sales brochure describing services or a screen shot of a web site (for services)

6. FILING FEES

-government filing fees $225 or $275 per class of goods/services, depending on the type of trademark application (if used a standard description of goods/services or a custom-made one).
